Requirements

1. All licensed professionals must be active and in good standing in the District of Columbia.
2. Proof of Identity (i.e. Driver’s License, Valid Photo ID, etc.)
3. Social Security card
4. DC/MD/VA Certification (+skills),  CNA | HHA | PCA
5. CPR & First Aid
6. TB (PPD) Screening Within six (6) months of application/Chest X-Ray (Valid for 5 Years)
7. Physical Clearance
8. Background Check
9. HEP B Vaccine Records / Declination Form
